UCDP Primary Number Change Form

Note: This form should only be used by approved Freddie Mac Seller/Servicers or TPOs who require a change of their Uniform Collateral Data Portal® (UCDP®) primary number (previously issued third-party originator (TPO) or Seller/Servicer number) to their current Seller/Servicer number. DO NOT complete this form unless you are an approved Freddie Mac Seller/Servicer.

Important Reminders: 

  • Users may be unable to access their UCDP profile and submit appraisals under the new number while the primary number change request is being processed. Processing can take 24-48 hours from when the request form is submitted. Please prepare accordingly
  • All UCDP Lender Administrators identified on this form should receive a new authorization email from the UCDP Team. Please ensure the contact information provided is up to date.
  • Be sure to reach out to your Freddie Mac representative or Onboarding Specialist for specific questions related to the impact of changing your UCDP primary number or call the UCDP Support Center at 800-917-9291.

Note: Please provide the contact information of all current UCDP Lender Administrator(s) within your organization. Any current Lender administrator(s) who are excluded from the list below will lose their UCDP Lender Administrator access under the new Primary Number. If you have questions regarding the status of your company UCDP Lender Administrator(s), please contact your Client Relationship Manager or send an email to [email protected].

  • I am an authorized representative of the Seller/Servicer listed above and I represent and warrant that I am duly authorized to submit this request.
  • I am giving permission for Freddie Mac to submit a request to change my Organization’s UCDP primary number (previous TPO number or Seller/Servicer number) to the New Seller/Servicer Number provided above for the listed UCDP Lender Administrators.
  • I understand that my organization’s UCDP profile may experience an interruption while the primary number change request is being processed. Processing can take 24-48 hours from when the request form is submitted and processed by Freddie Mac.
