Merged Credit Setup Form for CoreLogic Credco

Use this form to notify CoreLogic Credco of the following:

You are a Seller/Servicer and you intend to use Loan Product Advisor to obtain merged credit reports.
You are a Direct TPO and you intend to use Loan Product Advisor to obtain merged credit reports. If you know the wholesale lender to whom you intend to assign your loans, you are providing their Seller/Servicer number to authorize them to obtain reissues of your merged credit reports.
You are a Sponsored TPO and you intend to use Loan Product Advisor to obtain merged credit reports. You are providing your sponsor's Seller/Servicer number to identify them as authorized to obtain reissues of the merged credit reports you used to originate loans on their behalf.
